The latest version of the Vendor Request Form is posted to The FBMS Hub. The Hub is accessible by all DOI employees but must be accessed in Google Chrome at at https://sites.google.com/a/fbmsios.doi.gov/the-hub/?pli=1 . The vendor request form is located under Bureau Resources > MDM Vendor Requestmdm-vendor-request. BBI has also added a link to the Hub from the InsideFWS > FBMS > Business Process Guidance > Vendors.
